Achieve better care, better health, and lower costs by making high-quality cross-system care coordination available to all adults and seniors with complex needs in our region.
We invite professionals, para-professionals, and peers who provide care coordination for seniors and adults with disabilities in Medicaid long term care, hospital, primary care, specialty care, mental health, addiction, emergency, outreach and social service settings in Clackamas, Multnomah and Washington Counties to spend the day strengthening connections, discussing promising practices and skill building.
Join us for interactive workshops during our day to build up our skills. Mandy Davis, Dr. James Mason, and O'Nesha Cochran will join us as speakers, along with many esteemed colleagues from different systems leading workshops and conversations.
