As the Division Director for Aging, Disability and Veterans Services (ADVSD), I am excited to announce that we will be conducting a survey to assess the needs of diverse communities in Multnomah County. Your input in the area plan survey will be used to help us put together a Strategic Area Plan on Aging for 2025-2029.

We will be launching the survey on October 1st. Your valuable feedback will help us understand how ADVSD can enhance its programs and services to better support you and your community.

Take the Survey Today

The area plan survey is intended for Multnomah County residents and will be open from October 1st - October 25th, 2024. It will take around 15 to 20  minutes to complete. 

ADVSD is offering a $20 gift card incentive for Multnomah County residents who qualify. 

  • Note: only one survey can be completed per person and a maximum of two surveys per household (exceptions made for care settings). Gift cards will be available on a first come first served basis, available while supplies last.

    Eligibility:

    • Age 50+ (or older adults or elders as defined by their community)
    • People age 18+ with disabilities, veterans, and caregivers.

    Including:

    • Low income which includes food security, housing stability and access to health care, with particular emphasis on those with additional challenges due to physical/behavioral disabilities or chronic medical conditions.
    • At risk for placement in a long term care facility.
    • Historically underserved communities including indigenous people, immigrants and people of color.
    • LGBTQIA2S+ elders
