Note: To be eligible for enrollment on Kaiser medical and dental plans, the policy holder must live or work within the Kaiser-defined service area at least 50% of the time. Moving out of the Kaiser service area may result in becoming ineligible for the plan and necessitate enrollment onto our Moda medical/dental or Willamette Dental Group plan/s. Kaiser may update their service area mid-year (See list of current service area zip codes below).
