Community Health Specialist Series Positions in this series provide health information, advocacy, social support, and assistance in using the health care system to community groups and families.

Job description

EXAMPLES OF DUTIES:

  • Assist individuals, families, and communities identify strengths and needs and achieve goals in partnership with a Community Health Nurse or others; conduct selected health-screening procedures; maintain written documentation of activities.
  • Share culturally appropriate health education and information with individuals and families; assist in developing educational materials.
  • Guide individuals and families in learning the health care systems and accessing social services appropriately and effectively; provide social support for families and individuals.
  • Share community perspectives and needs with colleagues in the health and social service systems based on own life experience.
  • In addition to the above duties, the Community Health Specialist 2 will: develop and implement new activities, programs, and program evaluations; represent and promote programs to the community and other agencies; conduct outreach to specific communities; recognize and identify social and medical risk factors; develop and share interactive health education classes and presentations; coordinate the health care, psychological, and social services of selected individuals and families; make referrals to appropriate health and social service agencies; organize and conduct ongoing support and education groups in the community; participate in or chair health and social service committees; collaborate with other government agencies, organizations, schools, and community members to develop and implement activities; mentor Community Health Specialist 1s.
  • Perform other duties as assigned.

LEVEL AND TYPE OF WORK AND/OR SUPERVISION:

Community Health Specialist 1

  • Entry level
  • Technical
  • Perform the more routine duties as assigned with increasing independence.
  • Decisions made or actions taken require higher-level review and work is checked for progress and reviewed for quality and compliance to policy and procedures.
  • Typically, incumbents may use the entry level to obtain the proficiencies to qualify at the next level in the classification series.

Community Health Specialist 2

  • Journey level
  • Technical
  • Perform the full range of duties, working independently, applying well-developed subject knowledge, and exercising judgment and initiative.
  • Receive occasional instruction or assistance as new or unusual situations arise.
  • Fully aware of the operating procedures and policies within the work unit.
  • Work is reviewed based on compliance, appropriateness, and effectiveness in meeting goals and deadlines.

MINIMUM QUALIFICATIONS GUIDELINES:

Any combination of experience and training that would likely provide the required knowledge, skills, and abilities is qualifying.  Requirements are determined at the time of recruitment based on responsibilities of the individual position and business needs of the department.

Community Health Specialist 1

Training and experience for entry level, technical classifications are typically equivalent to an associate's degree and up to one (1) year of experience that demonstrates the ability to perform the duties of the position.  (Equivalency 2-3 years of qualifying training and/or experience).

Community Health Specialist 2

Training and experience for journey level, technical classifications are typically equivalent to an associate's degree and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.  (Equivalency 3-6 years of qualifying training and/or experience).

Depending upon assignment, candidates may be required to pass a criminal background check, have a valid driver license, and /or additional training, licenses, or certificates.

REQUIRED TRAINING, LICENSES, AND/OR CERTIFICATES:

Community Health Specialist 1

  • CPR certification is required.

Community Health Specialist 2

  • CPR certification is required.
