What Is ACHIEVE
ACHIEVE (Action Communities for Health, Innovation and Environmental Change) Coalition is a group of partners in different sectors who have a collective vision of ending health inequities in chronic diseases for African-Americans and African immigrants and refugees in Multnomah County. The coalition is facilitated by REACH and Upstream Public Health and consists of community organizations, faith organizations, community members and others who have a vested interest in Black and African community health.
History of ACHIEVE
Formed in 2009, the coalition has worked to improve health outcomes for the Black and African populations of Multnomah County. We prioritize community engagement and work with our partners to create local policy changes that benefit our communities. ACHIEVE has worked to increase access to healthy foods and make streets safer for pedestrians, bikers, rollers, and families. The coalition has also worked in tobacco cessation programming and policy and breastfeeding support in the places people live, work, worship and study. ACHIEVE coalition accomplishments over the years include:
- Enacting community policy and environmental assessments in over 60 community settings across jurisdictions
- Working with TOFCO and other partners to help enact the flavored tobacco sales ban in Multnomah County
- Co-facilitating a locally-sourced, CSA program with Play Grow Learn that has served thousands of families
- Publishing the 2021 Traffic Safety Report that led to awareness of disparities in casualties among communities, and increased funding for traffic control initiatives in high-crash corridors
- Receiving the Centers for Disease Control (CDC) REACH grant three cycles in a row (2013 to 2018, 2018 to 2023, 2023 to 2028).

Agenda Requests
You can fill out our ACHIEVE Coalition agenda request form in order to reserve a presentation slot at upcoming ACHIEVE meetings or to make a suggestion for discussion topics. We ask that you complete your form submission no later than the 3rd Wednesday of the month prior to the meeting you would like to present at.
