Client Satisfaction Surveys (DOH26-002): The survey indicates client satisfaction with contractor services. Includes original surveys and summary reports for each contractor providing services. Survey information includes information about the contractor, a description of services provided, number of persons served, and a breakdown of client responses.

Committee and Task Force Meeting Records (DOH26-003): Records documenting the proceedings of boards, commissions, task forces, committees, advisory councils, and other similar groups, as described in Oregon's Public Meetings Law (ORS 192.610 to 192.710), for which the department maintains the official record. Records may include minutes, agendas, exhibits, resolutions, staff reports, indexes, audio or visual recordings, correspondence, and related documentation.

Correspondence: Records that: 1. document communications created or received by an agency AND 2. directly relate to an agency program or agency administration AND 3. are not otherwise specified in the Multnomah County Retention Schedules. Records may include but are not limited to letters, memoranda, notes and electronic messages that communicate formal approvals, directions for action, and information about contracts, purchases, grants, personnel and particular projects or programs.

Quality Assurance Logs (DOH26-005): Series documents logs maintained to document quality assurance test have been made and recorded for clinic machines and procedures. Logs show machine or process name, date checked, results, and name or initials of person doing the check.
