Monitored Misdemeanor Program - Updating address and employment information

If you have two or more DUII Convictions in your lifetime you must apply for & receive permission to move to another state.

MCJRP/S.T.A.R.T Continued Care - Address and/or Employment Information Update

If you are moving, or there are any changes to your employment, you must report that to the Continued Care Supervision office within 48 hours.

If you have Police contact, you must report it to your PO.

Supervised Misdemeanor DUI Program - Updating Information Form

You must notify DCJ prior to/within 24 hours of changing residence. If you intend on moving out of Multnomah County – you must FIRST obtain permission from your Probation Officer.
